  • Passport Original, valid for the intended stay, 2 back-to-back blank pages for stamping — plus copy of all data pages, old passport (if any), and scan of first/last pages and all used pages of the current passport
  • Visa Application Form Completely filled and signed by the applicant
  • Family Information Form Completed in full (no blanks), signed by the applicant
  • Coloured Photograph 2 photos, no border, 35×45 mm, white background, 80% face cover — chin, nose, eyes, ears & eyebrows fully visible
  • Cover Letter On plain paper, stating name, designation, passport number, purpose & duration of visit, signed by the applicant
  • Invitation Letter From the invitee/host/relative, stating purpose & duration of stay

  Financial Document

  • Bank Statement (6 Months) Original, personal, mentioning bank name & telephone number
  • Income Tax Returns Last 3 years

  Employment / Income Proof — Choose One

  • If Employed 3 months' salary slips + NOC from employer
  • If Self-Employed Balance sheet of the company or partnership deed; registration proof / partnership deed / MOA; articles of memorandum (if Director/MD); brief company profile
